Mental Health Intensive Case Management Program
The Mental health Intensive Case management (MHICM) program provides outpatient, community care for veterans with serious mental illnesses. MHICM is a recovery-oriented program that provides multiple services such as crisis intervention, medication management, budget management, socialization skills, client advocacy, support and mental health education. The overall goal of MHICM is to provide veterans with mental health issues the ability to live with additional support in the community.

In order to qualify for MHICM services the veteran must be eligible for VA healthcare, have the diagnosis of a severe mental illness such as schizophrenia, bipolar or PTSD and have a history of 3 inpatient psychiatric hospitalizations within the past year, as well as be clinically appropriate to receive outpatient care services. After admission to the MHICM program the veteran will be assigned to a case manager who will provide weekly visits 1-3 times per week to provide intensive mental health case management services. Our MHICM team is made up of our Program Manager, Social Workers (LISW), Registered Nurses (RN), Peer Support Specialist, Recreational Therapist and a dedicated Psychiatrist. Our MHICM staff work closely with other medical and behavioral health providers to ensure quality coordinated care!
