Primary Care Teams - How to get in contact with the healthcare professionals on your Primary Care Team.
Programs and Activities - An alphabetical listing of the many programs and activities we offer to patient and family on their healthcare concerns. Your physician, nurse, social worker, dietitian, or other healthcare professional can refer you to a program or you can call directly to obtain information.
Community Based Outpatient Clinics - Contact to the closest Community Based Outpatient Clinic (CBOC) or Field Service Center serving veterans in your area of Southern Ohio. Each CBOC has a Primary Care Team consisting of a Doctor, Physician Extender, Nursing Staff and Administrative Support Staff. Your Primary Care Provider will coordinate your care, educational needs and any needed referral services. Field Service Centers offer health screening, assessments, and certain laboratory services as well as providing eligibility and enrollment information.
Veterans Service Officers (VSO) assist in filing claims, appeals and other legal issues pertaining to the Department of Veterans Affairs or the Military Services.
